APA Re-Write and source citations

I have a paper that I wrote, but got a low grade due to the lack of citations of sources. Here is what is wrong:

You have summarized the case study well but have failed to cite the source with
paraphrases and direct quotes that are taken directly from the NIOSH report. I
am going to give you the opportunity to re-write this case study using proper
APA format and in-text citations. Be sure to include a title page, body, and
reference pages with a running head and page numbers in the header. The case
study should be double-spaced throughout with the first line of every paragraph
indented. Be aware that the author’s last name and year of publication must match. For
example, you say Smith (2000) in the body of the essay but Smith-Terry, A. in
the reference section, then you must use Smith-Terry in the body of the essay
so they match. If a source is not used in the body of the essay with either a
direct quote or paraphrase, the source cannot be listed in the reference
section. In in-text citations: If a source does not have an author, then you
use the title of the article, book, or web page. (“Fire Behavior”, 2000) or
(“Fire Behavior”, 2000, para. 4).

I have attached the paper to be re-wrote using the correct APA format. I need it by Thursday evening.
