Need this Medical Form recreated to be editable and the exact same font and layout.

I have a document that i need done ASAP and it needs to be recreated so i can edit it through Microsoft Word. I need it to be done the exact way as shown and as close to the original as possible. Very easy job but just very time consuming for me. I have attached the file below. Thank You.
