Continuum of Care Services Review
Assume your role as a human service or public service leader for all assignments in this course. In this assignment, focus on gathering information about continuum of care services and writing a review of those services. The kind of information you will collect is relevant for a recommendations report and plan of action, which you will develop for your assignment in Unit 9.
This assignment includes three parts.
Part 1: Selection of Continuum of Care Services
Use the list below to select two continuum of care services provided for a diverse aging-adult population in your community:
- Prevention/primary care.
- Acute care.
- Long-term care.
- End-of-life care.
Part 2: Critical Review
Gather information about the continuum of care services in your community you selected. Do your research online, and follow up with a phone call if you need additional information.
Write a review of the services that includes a brief description of all of the following. Be sure to include how well you feel the facility offering the services is doing for each of the items. If the facility does not provide information on any of the points below, point that out in your review.
- Rights and responsibilities: The needs, rights, and responsibilities of individuals are explained upon entry into the facility.
- Transparency: All of the services provided are clearly stated and an appeals process is defined for residents/patients, families, and staff.
- Caregiver resource coordination: Professional, consumer, family, and other formal and informal caregiver resources are coordinated in the facility.
- Client-centered: The overall system in the facility is client-centered and cost effective.
- Finances: The facility is adequately financed.
- Access to regulatory reviews: The facility posts its reviews by regulators on its website and informs residents/patients, family, and staff when a regulatory review is conducted.
Part 3: Incorporation Into Action Plan
In Unit 9 you will submit a recommendations report and plan of action. For this assignment, identify a problem or issue that you learned about while researching specific organizations that could be the focus of a recommendation report and plan of action. Consider the information you gathered about the facilities (for example, rights and responsibilities and caregiver resource coordination) to identify a potential issue. Explain how you would incorporate the issue you identified into a recommendation report and plan of action.
